Position: Transition Services Facilitator

Reports to:  Director Student Services and Attendance, Mountain State ESC Executive Director

Qualifications:

  1. Bachelor’s degree in education, social work, human services field, or counseling
  2. Experience in student counseling, development, education, family support or related field may be preferred
  3. Understanding of the purposes and organization of public education
  4. Familiarity with school discipline policies, student support programs, courses and County policies
  5. Strong communication and interpersonal skills
  6. Be able to maintain a high level of ethical behavior and confidentiality of student information
  7. Willing to travel within the County to meet with students and families

Expectations and Responsibilities:

  1. Under the supervision of the Director of Student Services and Attendance, the Transition Facilitator will provide transition and support services to the Alternative Education Program (AEP)
  2. Meet with administrators to develop early interventions to ensure support strategies are implemented to address the student’s on-going behavior and discipline issues prior to referral to AEP.  Assist with developing Phase II behavior contracts.
  3. Coordinate student’s transition planning and placement meetings
  4. Prepare written and oral communication from the AEP to administrators and parent/guardians.
  5. Develop Individual Alternative Education Plans (IAEP) for ALL students assigned to the alternative program.
  6. Meets with students and school administrators to facilitate enrollment, resolve issues related to re-entry options for students as they return from the alternative classroom.
  7. Makes direct contact with parents and guardians to offer student support services
  8. Maintains case notes on AEP students and family contacts
  9. Must be willing to travel.
  10. Maintain a valid driver’s license
  11. Performs other duties as assigned by the Director of Student Services and Attendance, MSESC Executive Director, or other administrative staff

Salary:  $35.00 per hour (no benefits provided)

Term:  Successful applicant will work a minimum of 24 contact hours per week when school is in session and a maximum of 960 hours between the dates of August 23, 2022 and June 7, 2023.
