TITLE OF VACANCY:       SNAP E&T Case Manager, Part-time

POSITION TYPE:             Professional

REPORTS TO:  Executive Director

SNAP E&T Case Manager works with clients who have been referred by WV DHHR to participate in the Employment & Training Program (E & T).  The goal of the program is employment and the case manager works with clients on job readiness skills, career assessment and planning, as well as assisting with opportunities to complete GED or further their education to gain self-sufficiency.   Clients are provided with job leads, resume help, guidance for completing online applications and other services as needed.  The case manager stays in close contacts with participants and reports client progress to DHS monthly.

Responsibilities:

  • Accept voluntary referrals from DHHR for non-exempt Able-Bodied Adults Without Dependents (ABAWD) and other exempt SNAP clients who request SNAP E&T services;
  • Notify participants of appointments for orientation/enrollment, initial assessment, and development of the Personal Responsibility Plan
  • Screen and schedule participants for SNAP E&T activities;
  • Supporting and following up on SNAP E&T activities, including reviews of contact reports or participation documentation;
  • Enter data into the eligibility system for appropriate reporting for all SNAP E&T cases after referral;
  • Prepare information for DHHR for hearings and pre-hearing conferences and providing testimony as appropriate;
  • Notify the DHHR Worker when SNAP E&T participants are not complying with program requirements
  • Complete monthly report that lists SNAP E&T participants served during each month of eligibility and the amount of money spent for each participant.
  • Determine who should be referred to outside sources for training during the assessment process.
  • Make referrals to Vocational Training, Workforce Development Board, third-party partners, and specific local programs and other community services, as needed
  • Monitor the participant attendance
  • Maintain contact with the participant and service provider to ensure satisfactory progress is being made and to help eliminate barriers when needed.
  • determine the best means for the participant to achieve self-sufficiency, accept personal responsibility, and, if applicable, to meet the work obligation.
  • Establish reasonable and appropriate requirements, for the participant, related to the participant’s capability to perform the tasks on a regular basis, including physical capacity, skills, experience, family responsibilities, and residence. Reasonable and appropriate requirements must be based on the participant’s proficiencies and skills as assessed.
  • Monitor compliance progress to achieve self-sufficiency.
  • Provide continuous assessment of the participant’s needs, goals, and negotiates adjustments to the Self-Sufficiency Plan (SSP) as necessary.
  • Develops employment and other work activity opportunities for the participant within the community.
  • Provides payment for supportive services, i.e. transportation, as appropriate.
  • Identifies potential resources and makes appropriate referrals to access them.
  • Other duties as assigned

Minimum Qualifications:

  • · Baccalaureate Degree
  • · Excellent verbal and written communication skills
  • · Ability to effectively interact with participants and partner agencies
  • · Driver’s License and reliable transportation
  • · Self-motivator, ability to work with minimum supervision
  • · Excellent organizational and time management skills
  • · Knowledge of computer programs; specifically Microsoft Office Suite
